(HealthDay News) -- If you have a habitual sickness -- much as diabetes, arthritis or hunch disease -- you can verify an active persona in managing your health.

The American Academy of Family Physicians offers these self-management guidelines:

  • Examine your unwholesome behaviors, and opt which ones you'd like to change.
  • Create limited goals and an action plan for how you'll accomplish those goals.
  • Create backup plans -- for example, what you'll do for training when the weather is bad.
  • Feel overconfident in your ability to accomplish your goals; if they seem too tough, reconsider your goals.
  • Check in frequently with your student to discuss your progress and some problems.
